Trainer sign-off as an end-of-course requirement (Scottish licensing)

Scottish alcohol licensing customers (driver: Millennium) need their training certificate to carry the Personal Licence Holder's confirmation: the holder's name, personal licence number and issuing licensing board, plus an attestation that they delivered the training.
